We are looking for Nurse Practitioners to join our team in conducting patient centered concierge level care in patients' homes

The NursePractitioner makes up our patient intake team and are dedicated to onboardingeligible patients. The Nurse Practitionersare focused on completing the initial visit and assessment that begins thepatient’s longitudinal care with WellBe’s broader care team. In this role the Nurse Practitioner enganges the members and delivers primary care in the home-based settingfor patients. The Engagement Nurse Practitioners is a critical member of thecare team that will be accountable for not only engaging new patients, but alsoensuring we have a comprehensive medical record on file for the patient bycompleting accurate, timely documentation of the patient records forthese visits as part of our model that will support transitioning these new patients to our longitudinal care Nurse Practitioners. 

Competencies:

• Performs initial patient screenings/physical examinations/creates assessments plans
• Orders, interprets, and analyzes diagnostic tests and determines the need for follow-up appointments and further treatment options. 
• Delivers quality care while maintaining all company metrics pertaining to appropriate documentation.coding, HEDIS measure, engagement of patients and managing medical expense. 

Qualifications:

• Experience in family practice, geriatrics, hospice care, palliative care, hospitalist, ICU, SICU, ER 
• Minimum Master's Degree in Nursing 
• Valid nurse practitioner or physician assistant license/Board Certification 
• NPI number as an NP
• BLS/ACLS
• Valid DL, Auto Insurance, willingness to drive to patients homes/patient location
• salary $ 103,000 - $ 137,000 Plus bonus and full benefit package 4 weeks PTO amd more
• No covid vaccinations required
